Registration & Payment
• Each delegate must register online at www.sexualhealthconference.com.au
• No paper registration form will be available but if you are having problems with the online system
please advise the conference secretariat and an alternative system will be arranged.
• The registration cut-off date is Tuesday 2 September 2008. Registrations received after this date will be processed on site and will incur the A$55 late registration fee.
• Payment can be made by cheque, bank draft in Australian dollars (A$) or credit card (Bankcard, Visa or MasterCard). Cheques must be made payable to "ASHM"
• Payment must accompany all registration applications as reservations will not be confirmed unless payment is received
• Invoices are provided with your confirmation letter which are sent out periodically

Earlybird Rate
To qualify for the Early Bird rate your registration and accompanying payment must be received online by
5.00pm on or before Thursday 17 July 2008. . The Earlybird rate will not be given if payment is not received by that date and the standard rate will apply.
Student and Research Assistant Rate
To be eligible for the reduced student and research assistant rate all the following conditions need to be met:
• Students must be in full-time study (please attach a photocopy of your Student ID with your registration form)
• Registration must not be otherwise funded
• Supervisors must sign the appropriate section on the registration form
Trainee Registration Rate
To be eligible for this rate the delegate must be a trainee of the RACP.
AChSHM trainees whose abstracts are accepted for oral presentation at the conference will receive a refund from the Chapter of half the trainee registration fee. Trainees can apply for this refund after the conference.
Late Registration Fees
A$55.00 will be added to all registration fees (excluding day registrations) when registering after Tuesday 2 September 2008 or onsite.
Note: If registering late, we cannot guarantee the availability of function tickets.
Acknowledgement
Confirmation of registration, hotel bookings and payment will be made in writing until Tuesday 2 September 2008.
After this date, verbal acknowledgement can be obtained by calling the secretariat on +61 2 8204 0770. If you do not receive your acknowledgement please contact the conference secretariat on the above number. For registrations paid by organisations, please check with your accounts department to confirm that the registration form and payment have been forwarded to the conference secretariat.
Cancellations and Amendments
Registration and/or accommodation cancellations and amendments must be notified in writing to the secretariat. Cancellations received before Monday 11 August 2008 will receive a refund less A$150.00 administration fee. Refunds (less administration fee) for cancellations received in writing after Monday 11 August 2008 will be made only in exceptional circumstances. As an alternative to cancellation, your registration may be transferred to another person without incurring any penalty. The conference secretariat must be advised of the transfer in writing. Any outstanding refunds will be paid after the conclusion of the conference. A fee may be charged to your credit card by the hotel, according to their cancellation conditions when cancelling accommodation bookings after Monday 11 August 2008.
